Daily: after 8 hours in a day (1.5×) and after 12 hours (2×). Weekly: hours over 40 in a week that were not already paid a daily premium are also 1.5×. The two rules combine without paying two premiums on the same hour.

Each day earns 1 hour at 1.5× (5 premium hours); the 40 straight-time hours do not exceed 40, so there is no extra weekly premium: 40 × $20 + 5 × $30 = $950 gross.

8 hours at the regular rate, 4 hours at 1.5× and 1 hour at 2× — at $20/h that is $160 + $120 + $40 = $320 for the day, before any weekly calculation.

Yes — with a written agreement, overtime can go into a time bank and be taken as paid time off (at the same premium multiples) or paid out. The dollar value priced here is unchanged; only the timing differs.
